Coalition Health Access Program
The Coalition Health Access Program (CHAP) is
comprised of a network of health providers and volunteers in
Benzie, Grand Traverse and Leelanau counties. These individuals
and organizations donate many health services to uninsured
people so they may obtain basic health care. It is not an
insurance program, but has been created with donations and other
funds targeted for people who meet eligibility requirements.
Benefits of CHAP:
The benefits of being a CHAP member are that all CHAP members
have a card that allows access to:
a regular doctor, nurse practitioner or
physician assistant;
basic drugs (mainly generic) at pharmacies,
with a small co-pay;
labs, basic x-rays and EKGs at Munson
Healthcare facilities;
eye exam and eye glasses, with a small
co-pay;
referrals to specialists when ordered by the
primary care doctor, nurse practitioner or physician
assistant and scheduled by CHAP;
Assistance in working with Munson
Healthcare’s financial aid program, for some non-CHAP
covered items.
Eligibility:
To qualify for CHAP, persons must:
live in Benzie, Grand Traverse, or Leelanau
County;
be 19 years of age or older;
not have any other medical coverage or
insurance;
earn (before taxes and deductions) not more

Enrollment sites are in Benzonia, Lake Leelanau
